Champions Premier Dog Parlor prioritizes safety! We want to ensure that we have the proper authority to act on your dog’s behalf, should it be necessary. We also require that all pets who enter our locations show proof of a rabies certificate.
Please click the link below to view our General Dog Release Waiver. This waiver can be completed as an online submission or by downloading the PDF below. If you have a scanned copy of your Rabies Vaccine Certificate, attach it to the form. If you are unable to do so, bring the certificate to Champions Premier Dog Parlor prior to your appointment. This document will be added to your pet’s profile. Resubmission of this document for future appointments is not required unless Vaccine Certificate expires.

Your appointments are very important to the team members of Champions Premier Dog Parlor. We understand that sometimes schedule adjustments are necessary; therefore, we respectfully request at least 24 hours notice for cancellations by calling directly at 484-887-8800. Please understand that when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill that appointment time, and clients on our waiting list miss the opportunity to receive services. You will receive an appointment confirmation 48 hours in advance because we know how easy it is to forget an appointment you booked weeks ago. Since the services are reserved for your Pet personally, we kindly ask for 24 hours notice of a cancellation.
Cancellation Day of Appointment: 50% of the cost of service(s).
No call/No show: Full cost of the service(s).
